The WA Emergency Services Diligent Service Medal was introduced on 1 January 2005 by FESA to recognise employees who have served 15 years in the emergency services of Western Australia. A Service Clasp is awarded for every 5 years service thereafter. In August 2005, the FESA Chief Executive Officer, Mr Bob Mitchell, made the decision to include retrospective service to enable former employees who are eligible, to apply for the medal.

ELIGIBILITY CRITERIA
To be eligible to apply for the WA Emergency Services Diligent Service Medal you MUST have been employed for a MINIMUM of 15 years (accumulated service) with any of the following organisations:
Fire and Emergency Services Authority of WA;
Fire Brigades Board of WA;
Bush Fires Board of WA; or
State Emergency Service of WA.

(Note  where possible, applicants should attach any supporting
documentation as proof of service when applying)

In the case of eligible employees who have since died, the next-of-kin may submit a posthumous application. Should the next-of-kin not wish to receive the medal then, on that basis, the first application received with the written consent of the next-of-kin will be eligible to receive the posthumous medal. In the event of a posthumous application, only one (1) medal will be issued.

Any periods of leave without pay of 14 days or more will not be counted. (ie. the period of leave without pay will be deducted from the total years of service).

Volunteer service or employment with any other emergency service organisation in Western Australia or any other State or Territory cannot be counted towards this Medal.


APPLICATION PROCESS
Applications for former employees are open from 1 October 2005 and will close 28 February 2006. Application Forms are also available on the FESA website www.fesa.wa.gov.au or by contacting the FESA Reward and Recognition Coordinator on ph 9323 9520 or 9323 9491 or email rewards@fesa.wa.gov.au.

Applications received by the closing date will be processed by FESA to verify the eligibility of each application. Late applications will not be accepted.

Applicants will be notified in writing 8-10 weeks after the closing date as to whether their application has been successful or not.

Eligible applicants will be notified of the presentation date and venue where they will be formally presented their Medal.
